Notice to Employees: Availability of Form 1095-C

Employees are hereby notified that a copy of their Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, is available upon request.

If you would like to receive a copy of your Form 1095-C, you may request one at any time by contacting the Human Resources Department.  Requests may be submitted by phone, email, or in person to Paula Schwartze at 573-422-3304 option #1, or email pschwartze@viennaeagles.org or in person at 300 4th Street, Vienna, MO  65582. 

Once your request is received, a copy of your Form 1095-C will be provided to you promptly.
